Evidence integrity comes first
Reliable assurance depends on a complete, traceable account of the evidence. In care, assembling that account across fragmented systems is often the hardest part of the work.
The method
Care providers hold large volumes of operational and clinical evidence across systems with different structures, permissions and export rules. Its value depends on whether the right records can be located, attributed and placed in context.
GapScan combines a managed evidence-integration method with purpose-built Discovery and CareStory engines. Discovery establishes the evidence base; CareStory reconstructs what the records collectively show over time.
Who runs it
GapScan is operated by Rowan Visser MSc, a UK sole trader. Rowan leads the scoping, evidence investigation and client reporting for each engagement.
The work combines direct professional oversight with software designed to handle the slow, repeatable work of locating, validating and assembling records.
The method is designed for home care and aged care providers in the UK and Australia. Regulatory-framework mapping is offered only where it has been built and checked for the relevant jurisdiction.
What every engagement must preserve
- Source accountability. Every finding must be traceable to the record behind it.
- Human accountability. The auditor or responsible professional retains the final judgement.
- Evidential restraint. Conclusions stop where the available evidence stops.
- Controlled access. Only records authorised within the agreed scope are processed.
- Measured claims. GapScan does not claim accuracy or coverage that has not been demonstrated.
